The Orkneys and Shetland Islands were both part of the Norse Empire. The word "twatt" in old Norse means "a small parcel of land".
This just provides a wellspring of jokes: "You're a twat", barked a co-worker. "Why yes, I am. Are you? Orkney or Shetland?" she sweetly replied.
